The challenge we’re solving

In many European countries, children with complex chronic or palliative conditions depend heavily on hospital-based care. Families face emotional and logistical challenges, while primary care plays only a limited role. The result is fragmented care, and children miss out on the benefits of being safely supported at home.

HOME-C2P2 is building a new model of care – based on personalized home monitoring, shared care plans, and real-time coordination between local and specialist teams.

Together, we aim to shift care closer to families, reduce hospital stays, and improve life for children across Europe.

Project goals

  1. Identify what’s needed to implement the HOME-C2P2 model across different health systems.
  2. Co-design the HOME-C2P2 model with healthcare professionals and family caregivers.
  3. Study, from a multi-stakeholder perspective(professionals, patients, families, IT teams), the feasibility, logistics and acceptability of the HOME-C2P2 model in Spain, Denmark, the Netherlands and Slovenia.
  4. Prepare for HOME-C2P2 long-term integration into national care systems.

Our role

VOH.CoLAB contributes to HOME-C2P2 with expertise in participatory design, evaluation frameworks, and communication. Our responsibilities focus on ensuring that the voices of children, families, and healthcare professionals are systematically included, and that the project results are effectively shared across Europe.

Work Package 2 (WP2) – Co-creation and evaluation design

  • Designing all co-creation workshops in Spain, Denmark, and Slovenia, ensuring appropriate methods, tools, and wording for diverse stakeholder groups.
  • Creating the stakeholder engagement roadmap to guide participation across all sites.
  • Developing the evaluation framework in close collaboration with Erasmus MC (EMC), including meaningful PREMs (Patient-Reported Experience Measures) and PROMs (Patient-Reported Outcome Measures).
  • Supporting the co-creation of evaluation tools, such as surveys on quality of life, satisfaction, and expectations.
  • Providing methodological oversight for the pre-test phase, ensuring the framework and tools are practical, inclusive, and user-friendly.

Work Package 7 (WP7) – Communication and dissemination

  • Leading the project’s communication and dissemination strategy.
  • Preparing reports, news, and outreach materials to communicate progress and findings to families, professionals, and policymakers.
  • Ensuring results are widely disseminated at national and European levels, contributing to future models of care for children with complex needs.

Through these activities, VOH.CoLAB helps to align participatory co-creation with rigorous evaluation and guarantees that project outcomes are shared with a broad audience to inform policy and practice.

The impact we hope to create

Through cross-country testing, HOME-C2P2 aims to improve pediatric home care by enhancing coordination between primary and specialist services, using technology tailored to each child’s needs. This project is expected to generate the following impacts:

  • Better care at home: Fewer emergency visits, hospital admissions, and shorter hospital stays;
  • Empowered families: More coordinated support from local care teams;
  • More inclusive systems: Support for vulnerable families, especially in remote areas;
  • Lower costs: Reduced hospital use and more effective primary care;
  • Future-ready research: Data and experience to advance personalized prevention and AI;
  • Policy recommendation: Evidence to inform national strategies and care models for complex pediatric needs.
